Portfolio - Time stamp and User

Hi Team, 

Is it possible to date/time stamp portfolio items so that there is a clear record of when a document was added and the user that added/edited the document?

  Great idea. Even a log for the portfolio similar to the 'Student Log' at the bottom of the Units of Competency table would be great.
