Add Hyperlink direct to the student's contact page to Follow Up Reminder Emails

We think it would be so helpful if the Follow Up Reminder Emails that get sent from aXcelerate included a Hyperlink directly to the Student's contact profile, like the emails that are sent when a file note is emailed. :)
