Add record somewhere on a students file when assessments are deleted

Currently it seems there is no way to know if an online assessment or eLearning is deleted off a students profile. I'd like to see a system file note added similar to when a qualification is deleted, or a not is added to the student log under the linked qualification enrolment.

An 'are you sure you want to delete this assessment from Joe blogs' warning box would also be a nice to have feature as well.
