Add "Given, Middle & Surname" Fields to Certificate Templates

Would be very nice to have Given, Middle & Surname fields added into Certificate Templates for cources.  

Even if it was just available in the backend (using custom text), and not nessessarily added to the drop down list that would be fine.  

Would like to have [Participant Given Name]  [Participant Middle Name] on one line, and 
[Participant Surname] on another line.

  Also [Date Completion Expected] would be a significant addition.

  ... The reason being that [Date Completion Expected] is particularly important for setting the expiry date for ID Cards printed in bulk using the course Certificate Templates.
