in-house quotation

Unless i've missed it, the in-house quotation feature seems to be missing some fields during the setup which used to be there. I'm not sure at what point they were removed but I would like the see the following added back in as optional fields:

- Trainer
- Date/s
- Number of participants (min & max with group pricing options)
- Option to choose whether to quote per participant rate (currently I can only see a per day, hour, session rate)
