Move USI Field and add system note for verification

The USI field is critical for compliance, it should be located in a clearly visible place on the contact page so that it is not overlooked - in the Important Information section near the top would be appropriate.

It would also be useful to have a record of who verified the USI and when. A simple system generated contact note "USI Verified/Created by UserName" would suffice.
