Forms / Online Enrolment Info Capture

We are using Forms for our Onboarding/Enrolment Form, we have Online Enrolment Info Capture set up. However the Learner Declaration, as in the text the students are signing they agree to, doesn't show up on the Online Enrolment Info Capture. If this went to audit there is no way for us to prove the student actually signed they agree with our Terms and Conditions.

I am hoping a field for Enrolment Terms and Conditions can be added to Forms which will display in the Online Enrolment Info Capture, or another suitable solution.
