Provide an 'Include middle name on certificate' button or tick box when issuing certificates rather than current system or editing templates

Currently to include a middle name on a certificate, we have to edit the certificate template(s).

We do print version for pre-printed paper and digital email certificates, so this means going to 2 certificate templates, changing them both to include the middle name, issue the certificate for that student, then change the templates back and hope no one else in the organisation was issuing a certificate on either of those templates at the same time and that you don't get interrupted during this process and miss changing them back.

It would be so much easier if we could just select a setting when we are issuing the certificate to include the middle name for that specific student or cohort, rather than having to adjust the template.

It is rather painful with the current system.
