Information Blocks for Onboarding Forms

It would be good to have information blocks added to the forms - outside the headers and the descriptions even without it being a required field in the form. 

If we could add a description for the fields they are completing that would help clear the information for candidates. 

We have a lot of candidates completing the Preferred Name section within the form with nicknames or re-entering their given names.
