Enrolment capture from Onboarding missing logos and important information

The onboarding enrolment capture needs more information on it eg: the company logo and the declaration, so I can use it as an enrolment form. The old online enrolment form that we used to capture from the website was great but the onboarding one has no logo and the declaration is missing. We want to use the onboarding but need it to match what the old enrolment form looked like for ASQA.
