Observations in assessments

I am looking for ideas on how to include observations either as a task within a students submission evidence assessment as an assessor only tool, or as a separate assessment which students can see but not access allowing assessors to comment and select a Y or N kind of checklist.
If you have achieved this or have any ideas, please let me know.
